Our Services

At Hawkins Fall Prevention and Home Assessments, I provide a comprehensive in-home assessment that evaluates fall risk, functional mobility, and the ability to safely perform daily activities within the home. These assessments are grounded in the specialized training of occupational therapy and the CDC's STEADI program. Services are tailored to each individual’s goals, abilities, and environment, with advanced expertise in fall prevention and aging-in-place design to support safe, independent living at home.

Every Assessment Includes

  • Fall risk screening using evidence-based tools

  • Evaluation of mobility, balance, and transfers

  • Review of current functioning with everyday tasks and activities, such as:

    • Bathing and showering

    • Toileting

    • Cooking and kitchen tasks

    • Laundry

    • Cleaning

  • Home walkthrough to identify safety risks and accessibility barriers

  • Consideration of:

    • Medical history and current health status

    • Assistive device use

    • Medications (as they relate to fall risk)

    • Cognitive and sensory considerations

    • Caregiver support

  • Education provided during the visit

  • A personalized written summary with suggested home modifications, equipment, and fall prevention strategies

How This Assessment Is Different?

This assessment is conducted by an occupational therapist with specialized training in fall prevention and aging-in-place design. Occupational therapists are uniquely trained to evaluate how people function within their environments and to identify practical, evidence-based solutions that support safety and independence. Recommendations are guided by a person–environment–occupation approach and tailored to the individual’s goals, abilities, and home.

What Happens After the Assessment?

Following the assessment, clients receive a written report summarizing findings and recommendations. Follow-up visits are encouraged to review outcomes, adjust strategies, or provide additional guidance. If home modifications are completed by a contractor, I can remain involved as a functional consultant to support safe and appropriate implementation.

Pricing Structure

Comprehensive Home Safety & Function Assessment: one flat rate to be decided before the start of services (Includes assessment of one individual and one home) Additional Individual Assessment: additional fee per person (Applied when more than one person’s fall risk, mobility, or ADLs are assessed during the same visit) Follow Up visits and ongoing consultation with contractors available for an additional cost
